Tensions run high at Colony House when word of the coming food shortage leaks out. A night at the clinic takes a terrifying turn.

Boyd confides in Tian Chen that he did what he did with Sara for the good of the town, but Kenny interrupts and makes it clear he still does not trust Boyd.
At the church, Boyd finds the place emptied out and is confronted by a strange ballerina who appears to dance to a music box tune. The encounter turns horrifying when the ballerina starts expelling worms from her mouth, and Boyd begins experiencing more pain and hallucinations tied to the same eerie music-box imagery.
At the clinic, Kristi is dealing with her own personal stress when she confronts Marielle about relapsing after noticing she has taken some of the morphine supply. While Kristi tries to care for Boyd, she also discovers something deeply disturbing in his condition: strange creatures seem to be inside his blood, and worms are visible under his skin.
Meanwhile, the food shortage at Colony House starts becoming impossible to hide, and when the news leaks, tensions explode among the residents. Donna tries to manage the dwindling supplies with help from Tian Chen and the Matthews family, but the pressure over rationing causes anger and accusations as people fear there will not be enough food to go around.
The unrest leads to a serious accident at Colony House that leaves Ellis badly injured and fighting for his life. Elgin and Fatima work together to get him to the clinic while monsters begin to gather outside, and the group manages to bring Ellis inside for emergency treatment.
At the clinic, Kristi determines that Ellis needs a blood transfusion, but Boyd refuses at first because he fears whatever is in his body could be passed on to Ellis. Kenny pushes Boyd to make a choice, and Marielle and Kristi both see the worms beneath Boyd’s skin, confirming that his condition is far worse than anyone first realized.
Boyd ultimately decides to act. He goes outside and deliberately draws the monsters toward him, then cuts his hand and attacks one of them, using his blood in the struggle. One of the monsters appears to die after contact with his blood, and the others watch with surprise, suggesting Boyd has discovered something important about the infection.
Near the end, Donna and Ethan talk privately, and Ethan admits he lied to his parents and is really scared. The episode closes with Ellis waking up, leaving the town shaken but briefly relieved after the crisis.
