Kiyotaka Ijichi, assistant manager at Tokyo Jujutsu High, drives first-year students Yuji Itadori, Megumi Fushiguro, and Nobara Kugisaki to a detention center quarantined under the guise of a gas leak, where a **cursed womb** has been sighted through the windows. He explains that five detainees remain trapped inside Detainee Block 2, and the womb could evolve into a **special grade cursed spirit**—a level comparable to the strongest sorcerers, like their teacher Gojo who is currently away—with their only options upon encountering it being to run or die; their mission is strictly to verify survivors and rescue them if possible.

Outside the gates, a distraught mother pleads for her son Tadashi among the prisoners, fueling the trio's resolve as Ijichi erects a veil to conceal their entry from the outside world. Inside, the building warps into a curse's **innate domain**—a distorted, fleshy realm where the exit door vanishes—and Megumi summons his **White Divine Dog** to sniff out threats while they search for humans.

The group discovers the mutilated corpses of the five detainees, including Tadashi, and Yuji insists on retrieving the bodies but Megumi refuses, prioritizing escape as the domain pulses with growing menace. Two evolving curses emerge from the womb, transforming into a powerful special grade entity with multiple faces and limbs; the trio fights back with Yuji wielding the cursed tool **Slaughter Demon**, Megumi's shikigami, and Nobara's hammer and nails, but the curse overwhelms them, severing Yuji's arm and separating Nobara into a sub-realm swarming with smaller curses.

Yuji, battered and pierced by cursed energy blasts that burn his fingers, hits a breaking point and internally pleads with the entity **Sukuna**—the malevolent force residing within him—to take over his body in exchange for saving his friends. Sukuna agrees but only if Yuji dies afterward, mocking his weakness before emerging with tattoos, extra eyes, and a devious grin; he effortlessly dominates the special grade curse, healing from its attacks while insulting its rank and demonstrating true power by activating his **Domain Expansion: Malevolent Shrine**, which instantly shreds the curse into pieces and allows him to reclaim a **cursed finger** that had spawned the womb.

Megumi rescues Nobara from being devoured using a summoned frog shikigami, and they flee the collapsing domain as Ijichi expands the evacuation radius; Nobara, gravely wounded, is rushed to the hospital while Megumi waits outside for Yuji, requesting Grade 1 sorcerer backup amid uncertainty about Yuji's fate inside with Sukuna. The episode closes on Sukuna taunting the defeated curse remnants.
